Managing long term medical conditions
One condition, many moving parts. Your GP ties it all together.
Chronic conditions such as diabetes, heart disease, chronic pain, and respiratory illness require more than medication alone. They require an ongoing, coordinated approach to care. That's where a GP Chronic Condition Management Plan (formerly known as the GP Management Plan) comes in.
Through this structured plan, your GP works with you to set clear health goals, arrange necessary tests and reviews, and refer you to allied health professionals, including physiotherapists, dietitians, exercise physiologists, and podiatrists, as part of a well-rounded management strategy. Eligible patients can access Medicare-subsidised allied health visits, helping make this multidisciplinary care more accessible.
But the plan doesn't end with referral.
Your GP continues to play a central role. Monitoring your progress, reviewing test results, adjusting treatment where needed, and helping your care remain joined-up and effective over time.
Chronic disease management isn't just about prescriptions. It's about proactive, coordinated care, with your GP at the centre, guiding your health journey every step of the way.
